MCC eLearning Request for Traditional Course Test Scheduling

This form should be completed once by a traditional course instructor for use by the Holladay Center for eLearning to schedule class tests for a single semester. Please ensure that you have verified each course, CRN, section, testing date and time, etc., for each appointment before submitting this form.

Class tests are scheduled on a first come, first served basis, AND cannot interfere with online midterms and finals.

Completing this form does NOT guarantee testing date/time. Please wait for confirmation from an eLearning staff member that the appointment(s) is confirmed.

*Please Note: Instructors are required to come with their class and remain until each student is finished testing.
